This week, CMS
posted a notice on the skilled nursing facility (SNF) quality reporting program (QRP) website of an issue
identified with the SNF Review and Correct Report. Per CMS, if one or more
modification records are submitted for a patient stay, the most recent
submitted assessment, which was accepted by the Assessment Submission and
Processing (ASAP) system on or prior to the submission deadline for quarters
1 and 2 of CY 2017, it is not being used in the measure calculation.
Therefore, the measure results on the SNF Review and Correct Report are not
being updated to reflect any changes in the measure outcomes based on the most
recently submitted assessment. This applies to all measures on the SNF Review
and Correct Report. Please note that refreshes on the calculations occur on
Mondays.
A second issue has
been identified in the coding of one measure contained in both the SNF Quality
Measure (QM) and Review and Correct Reports that affects the calculation of the
measure, Application of Percent of LTCH Patients with Admission and
Discharge Functional Assessment and a Care Plan that Addresses Function
(NQF#2631). As a result, the data is currently being recalculated and
retested for this measure in both reports. The SNF QM report for SNF QRP is not yet available and is anticipated to be released in
late fall 2017.
CMS staff are
working to resolve these issues. A follow-up message will be sent by CMS as
soon as these are resolved and further guidance can be given.

SNFs should
continue to review and correct assessments where appropriate. Until this fix is
fully implemented by CMS, modification records will not show in the SNF Review and Correct Report.
